The UK HRT prepayment certificate
There's a specific NHS certificate that exists just for HRT, separate from the general prescription prepayment certificate — and it's easy to miss if you're not looking for it by name. It covers unlimited HRT items for a year for one upfront fee, and for most people collecting more than two HRT prescriptions annually, it works out cheaper than paying per item.
What it actually covers
The HRT prescription prepayment certificate (HRT PPC) covers every item on the NHS's specific list of HRT medicines, unlimited in quantity, for 12 months from one flat payment. It's separate from — and, for HRT specifically, generally cheaper than — the standard prescription prepayment certificate, which covers all NHS prescriptions but costs more.
The arithmetic, in general terms
Each individual NHS prescription item carries a standard charge. The HRT PPC is priced to cost roughly the same as two of those individual charges, covering the whole year regardless of how many HRT items you actually collect. That means:
- Collecting HRT once or twice a year: paying per item may cost about the same or less.
- Collecting HRT three or more times a year (common if you're on more than one product, or collect monthly): the certificate is very likely to save money, since every collection beyond the second is effectively free for the rest of the 12 months.
Check the exact current figures on the NHS Business Services Authority's page before deciding — prescription charges and certificate prices are reviewed periodically, and a number quoted today may not hold a year from now.
If you take other regular prescriptions too
The HRT PPC only covers items on the HRT list. If you also collect regular prescriptions for something unrelated, compare against the general 3-month or 12-month prepayment certificate, which covers everything — for some people that's the better value overall even though it costs more upfront than the HRT-only certificate.
What to log
This is one of the few genuinely practical, non-clinical things worth noting in an HRT record: the date you bought the certificate, and a rough count of how many HRT items you actually collect across the year — useful the next time it's due for renewal, to check whether it's still the right choice for your regimen.
