When an HRT patch won't stick
A patch that keeps lifting or falling off is one of the most common practical complaints about transdermal HRT, and it usually has a fixable, mechanical cause rather than being a sign the patch itself is the wrong choice.
What usually causes it
- Skin oils, lotion, or sunscreen underneath the patch, which interfere with the adhesive.
- Sweat and heat — a patch applied before exercise or in hot weather is more likely to lift.
- Friction from a waistband, bra strap, or tight clothing over the site.
- Reusing the same spot too soon — skin needs time to recover between applications, and adhesive doesn't hold as well on already-irritated skin.
What actually helps
- Apply to clean, dry, cool skin with nothing else on it — no lotion, sunscreen, or (commonly missed) alcohol wipes, which can break down the adhesive rather than help it.
- Press firmly for 10 to 15 seconds and run a finger around every edge to confirm it's sealed, not just stuck in the middle.
- Avoid the waistband and high-friction areas. The lower abdomen or upper buttocks are the areas most commonly recommended.
- Rotate sites so the same spot isn't reused within about a week — this protects both adhesion and skin condition.
What to do if it comes off
If it's still sticky and it's been less than roughly 24 hours, it can usually be pressed back on or replaced with a fresh one on the same schedule. If it's lost its stickiness or been off longer than that, replace it with a new patch and continue your regular schedule from that point — rather than trying to calculate and "make up" the missed hours, which isn't necessary and isn't something to guess at.
What to log
A patch coming loose is worth a dated note, not just a private annoyance — a pattern of repeated adhesion problems is useful information for a review appointment, and different from a genuine tolerability issue with the medication itself. Record: the date, roughly how long it had been on, and what you changed afterward (site, technique, or nothing).
